Eligibility:

Below 100% of federal poverty guidelines
Current proof of income, for 4-weeks

Services provided:

Comprehensive health services
Developmental and health screenings
Parent education and parent-child activities
COR Advantage Learning Assessments

Center-Based:

Infant and Toddler High Scope Curriculum
Classroom setting
Nutritious meals

Home Based:

Partners for a Healthy Baby Curriculum
Weekly home visits
Biweekly socialization activities in Plainfield and Somerset

Eligibility:

Below 100% of federal poverty guidelines
- Current proof of income, for 4-weeks

Expectant Women

Services provided:

Advocacy for healthy and nutritious life styles
Early and continuing risk assessment
Mental health interventions and follow-up
Prenatal education & postnatal assistance
Assistance with entry of baby into Infant Toddler program (depending on availability of space and income eligibility)
