making a transportation appointment inform HOPES
Inc. staff if the transportation service will be a one-way or a two-way
trip as well as if a wheelchair accessible vehicle is required. |

On
the day of the appointment, please be outside at the scheduled
pick-up time. |

All
clients requesting a return trip home must call the HOPES
Inc. office at the end of the appointment. |
|
To
change an appointment time, cancel an appointment, and/or
cancel a scheduled return trip from a medical appointment,
please call the HOPES Inc. Office. If a staff member is
not available to receive the call, please leave a message. |
